Community Occupational
This service is jointly funded between Rotherham Primary Care Trust and Social Services. Occupational Therapists are based within the Older People's Teams in the three Social Services localities. Also Occupational Therapists are based centrally with the Physical Disability and Sensory Impairment team. They work closely with other health professionals and Social Workers within these localities.

The aims of the services are:
- to enable people with physical disabilities of all ages to remain in their own homes as independently and safely as possible.
- to reduce the risk of admission to hospital through the provision of equipment, adaptations, advice and rehabilitation.
When a person is discharged from hospital and requires ongoing support to enable them to remain at home. Community Occupational Therapists liaise closely with Occupational Therapists based at Rotherham General Hospital.
