We work with Occupational Therapists both in the Community and in Health, Social Workers, Mental Health Services, Children and Young Peoples Service and the Learning Disability Team to ensure that all assessments and recommendations are made using a multi-agency approach.
How do I apply for medical priority housing?
You will need to complete a self-assessment form, you can download a copy here.
Self Assessment Form (112kb). Alternatively contact our team or any 2010 Housing Office and we will send one to you.
What happens when I have completed the form and sent it to the Assessment Team?
You will receive an acknowledgement letter within 10 days. If you are eligible, we will then send you an appointment for the Assessment Officer to visit you at home to carry out an assessment of need. If you are not eleigible we will write to you and tell you why.
Do I need to be on the Housing Register?
Yes. If you aren't already registered, you can register on line using our
Housing Register Application eform.
Can I keep my General waiting list date if I am given a Medical Priority and a place in the Priority Group?
No. Your General application will be cancelled as soon as you are awarded a Priority and this will be dated from when you were assessed
If I need specific equipment in the property for my disability who would provide this?
The Occupational Therapist involved in your case would ensure that any equipment needed is provided when you take up your tenancy. Any minor adaptations e.g. grab rails, extra stair rails to ensure your health and safety will also be addressed
Do I need to get a letter from my doctor, social worker or support worker before I can apply?
No. The Assessment Officer who is allocated to your case will seek further information from other professionals if needed
