Personalisation can help you to receive services designed specifically for you, at times and places to suit you. You know what you need and how your needs can best be met.
Personalisation can give you more choice by providing you with access to information and advice about support and services available to everyone locally including things like transport, leisure, education, health, housing and community safety.

How can I get more choice and control?
Your first step is to get a social care assessment around what you can do for yourself and where you need some support. You will be assessed against the Council's Fair Access to Care guidance which will determine your level of need.
Your Social Worker will make sure you have access to the right information, advice and support to help you choose serices that are right for you with the money from the council.
