Medical Priority Housing
The Assessment Team provides a comprehensive assessment service to all vulnerable customers who need to apply for priority housing.
You can apply for Priority housing by completing a Self-Assessment Online Form.
Our main partners who we work in partnership with to identify the type of housing needed to meet individual needs are:
- Adult Services
- Health
- Learning Disability Service
- Mental Health Service
- Voluntary Services including housing related support
To also support our assessments and recommendations for Priority Housing we have an Occupational Therapist based in the team and also work with other Occupational Therapists across the borough
How do I apply for Medical Priority housing?
By completing an online self assessment form or by contacting the Assessment Team on 01709 823468.

What happens next?
Receipt of your self-assessment form will be acknowledged by mail immediately followed by a letter within 3 working days. If you are eligible for an assessment we will send you an appointment for an Assessment Officer to visit you at your home to carry out an assessment of need.
If you are not eligible, we will write to you and tell you why
What does the Home Assessment consist of?
The Assessment Officer will assess your housing need by asking you questions in respect of how you manage your day to day activities in your present property. The Officer will also need to be informed of any medical problems you may have.
At this assessment you will also be given advice in respect of any benefits you may be entitled to, housing related advice in respect of types of properties and areas and also support offered on a weekly basis if needed in respect of placing expressions of interest for properties advertised
You will also be given the direct contact numbers at this visit for your named officer to enable you to contact them at anytime to discuss your case.
Do I need to be on the Housing Register to receive an assessment?
Yes. If you are not registered you can register on line using our Housing Register Application Form.
Can I keep my General waiting list date if I am given a Medical Priority and a place in the Priority Group?
No. Your General application will be cancelled as soon as you are awarded a Priority and this will be dated from when you are assessed
What if I need special equipment to help with my disability?
The Occupational Therapist involved in your case would ensure that any equipment needed is provided when you take up your tenancy. Any minor adaptations e.g. grab rails, extra stair rails to ensure your health and safety will also be addressed at your present property
Do I need to get a letter from my doctor, social worker or support worker before I can apply?
No. The Assessment Officer who is allocated to your case will seek further information from other professionals if needed
How do I place expressions of interest on vacant properties that are being advertised?
The Key Choices webpage states lots of information on how you can request a property. You can also contact your assessment officer who will make a request on your behalf.
