Out of Hours - Social Care
The Out of Hours Social Worker Team will deal with appropriate health and social care crisis or emergency situations that occur in the evening or over the weekend to minimise risks and ensure safety.
We will:
- Be accessible via the telephone 24 hours a day (01709 336080) where a social worker is available out of hours up to 10 pm weekdays and at weekends to provide an emergency response to your situation.
- Work in partnership with fast response nurses and staff in A and E to ensure that an appropriate support package is put in place to meet your immediate health and social care needs.
- Act upon any concerns you express about feeling vulnerable to ensure your safety and undertake further enquiries regarding potential safeguarding concerns. On confirming safeguarding action is required and where necessary, will immediately make safe the vulnerable adult; or liaise with the safeguarding team the next working day as appropriate.
- Determine that if your situation does not require an immediate response, any contact details taken via the telephone or in person will be followed up by an emergency duty social worker the next working day.
- Ensure an acknowledgement is made by telephone from a Social Worker the next working day who will be able to explain what will happen next.
