Health and Social Care Out of hours service standard
What you can expect from us . . .
The 'Out Of Hours' team will deal with appropriate health and social care crisis/emergency situations that occur in the evening or over the weekend to minimise risks and ensure safety.
The Council's 'Out of Hours' team will:
- Be accessible via the telephone 24 hours a day (01709 364689) where a social worker is available out of hours up to 10pm weekdays and at weekends to provide an emergency response to your situation. (new standard)
- Work in partnership with fast response nurses and staff in A and E to ensure that an appropriate support package is put in place to meet your immediate health and social care needs. (new standard)
- Act upon any concerns you express about feeling vulnerable to ensure your safety and undertake further enquires regarding potential safeguarding concerns. On confirming safeguarding action is required and where necessary, will immediately make safe the vulnerable adult; or liaise with the Safeguarding team the next working day as appropriate. (new standard)
- Will determine that if your situation does not require an immediate response any contact details taken via the telephone or in person will be followed up by an emergency duty social worker the next working day. (new standard)
- Ensure an acknowledgement is made by telephone from a Social Worker the next working day who will be able to explain what will happen next. (new standard)
