Example of how a young person can be supported to develop independence skills

This can be seen as a 3 step process moving young people from their care setting to independence through managed and targeted planning. Progression through each step should be achieved within a six month period although detailed assessment and planning will take into account each Young Person’s individual needs and abilities.

The objective of this programme is to develop the skills for independent living within a twelve month time frame.

Prior to starting the programme, a clear plan will be agreed between the provider, LAC and Leaving Care Service and the young person, setting out an assessment of abilities, targeted outcomes and measures. Throughout the programme the Young Person will be supported by their named support worker and their social worker or Personal Adviser. Monthly reports will be provided from the home to their allocated lead worker outlining progress, achievements and needs, including amended planning agreed between all parties.

Planning and achievements will link to the Young Person’s Pathway Plan.
