What happens if we decide that you are at high risk or very high risk?
We will listen to your views and try to plan your help and support around your wishes wherever possible. If you tell us that you prefer to stay in your own home then we will make every effort to make that possible. In some cases we may decide that your needs would be best met in a care home. When we agree your care plan with you, we will also think about your cultural and religious needs. When we have agreed what help and support you will receive this is written in your care plan. You will be given a copy of your assessment and your care plan.
If we decide that you are at high risk (Substantial) or very high risk (Critical) we will offer you help and support.
