What will Personalisation mean to you?

Personalisation means that you should receive services designed specifically for you, at times and places to suit you. You know what you need and how your needs can best be met. Personalisation is about giving you the information and support to make your own choices about what you need.

Personalisation means making sure that you know about the services that can help you. You will have access to information and advice about support and services available to everyone locally including things like transport, leisure, education, health, housing and community safety.

Personalisation means that if you are a carer you will get the help that you need. You will be recognised in you role as a carer and supported in that role. You will also be supported to have a life outside your role as a carer.

You will have support to assist you if you need a little more help, at an early stage so that you can stay independent for as long as possible You will be able to choose who provides the support that you need, and have control over when and where the services are provided.

You will have access to the right information, advice and support to help you decide so that you can make you own plans for services with the money from the council or ask us to arrange things for you.

Personalisation will help everyone who needs care and support, for further details please contact Assessment Direct.

Personalisation means that you will have the opportunity to be part of your local community and experience the friendships and care that can come from families, friends and neighbours; without putting an unreasonable burden on friends and family who want to help you. Personalisation is a new way to think about how you get care and support. In the past people were expected to fit into services. You will still need an assessment of what you can do for yourself and where you need some support. We will still use the same Fair Access to Care Services guidance to decide who needs help the most and what care and support we can give.
