Why am I having a review?
We need to make sure the level of support is meeting your needs so that you can continue living independently for as long as possible.
We will ask you questions about how you are coping with your current level of support to do your daily living tasks. This might include things such as:

Your health and mobility (eg. Details of any physical conditions, mental health conditions or other long-term conditions)

Your personal care (eg. How you cope with washing, dressing and toileting)

Daily living tasks (eg. Making meals and drinks, shopping and laundry)

The people who care for you (eg. Family, friends and neighbours who support you)

Your emotional needs and your social activities (eg. Contact with family and friends and community involvement)
