What will happen to my care package?
We will discuss the care you are currently receiving with you. Your family, carer or a friend can also be present if you would like, so we can jointly decide if your current care package is still meeting your needs.
Your care package may remain the same or it could be increased or decreased depending whether there has been any changes in your needs.
We will then send you a copy of your new care plan that we have agreed with you, detailing any changes in the level of support you will be receiving.
