This guide was created in partnership with parents, carers and partner organisations to create a useful resource for parents/carers with young people with special educational needs and/or disabilities (SEND).
This guide sets out how services in Rotherham can work together to support young people with special educational needs and disabilities to have a successful transition from childhood to adulthood.
The following pages aim to help you guide your young person into independence, or as much independence as is possible for the young person. It will give you information about what to think about, plan and do, and what to expect during this time as the young person becomes an adult.
The guide is designed to help support you through the Preparing for Adulthood and transitions process in Rotherham when your young person reaches the age of 14 or is in Y9 up to the age of 25.
Some of the most important things the guide will help you think about is:
- the transitions journey - what to expect and when
- access to community and things your young person enjoys
- education, training and employment
- health and social care
- independent living
