Your Pathway Plan – Your Future, Your Way (Ages 16-25)

Your Pathway Plan is your personal roadmap for the future. It sets out your goals, the support you will get, and when things will happen.

At 15 years and 9 months

Your Pathway Plan begins. This is your personal plan for the future.

  • part 1: Your needs – things you might need support with (like housing, health, money)
  • part 2: Your goals – what you want to achieve, how you’ll get there, and who will help

Your first plan will be finished when you are 16 years and 3 months. From then on, your plan will be updated regularly:

  • at least every 6 months
  • anytime something big changes (new home, new job/college, health needs, or family changes)

Age 16-17

You will still have a Care Plan and a Pathway Plan

Your PA and Social Worker will both support you with this.

Your plan will be reviewed every 6 months minimum, and sooner if things change.

Age 18-20

Your Pathway Plan continues and is updated regularly to make sure it still reflects your goals and needs.

Reviews happen at least every 6 months.

Age 21-25

You can choose to continue having a Pathway Plan if you want extra support for example with housing, health, work, or family. It is your choice. The support provided will be based on an assessment. If you contact your PA or the Duty PA they can explain more about the offer to you from 21-25 years.

If you choose not to, you can still return at any time for a new plan and PA support.

Even if you don’t have an allocated PA, we will:

  • check in with you once a year to see how you’re doing
  • be there if you drop into The Journey Hub
  • celebrate your achievements with you – whether it’s a new job, passing your driving test, having a baby, or anything else important to you
  • Previous page: Visiting and Contact Aged 16 to 25
  • Next page: Extra support you can expect
