How to Access Support
Pre 21 Planning
A final pathway plan will be completed with you before 21 to summarise your achievements, support needs, and arrangements.
It will tell you how to access support up to 25, your wishes for future contact, and preferred contact methods for example phone, text, face to face.
If you decline support, we will still try and contact you annually by call, or visit.
