Make a Referral to the Inclusion Pathway Panel
Before you submit a referral, you will need to provide the following:
- Parental consent
- Details about the parent or carer (including a second parent or carer, if relevant)
- Information about the child or young person, including:
- Personal details
- Education and assessment history
- Medical information
- Details of any professionals currently involved
