Make a referral to the Medical Home Tuition Passport
Before you proceed, you will need to provide the following information in your referral:
- A letter or report from a clinical consultant confirming the child’s medical condition and recommending home tuition.
- The expected timeframe for tuition, as stated in the medical evidence.
- For mental health absences:
- Self-certification for the first week (if applicable).
- Medical evidence from an appropriate professional (e.g. CAMHS consultant) for ongoing absence.
- The child’s school details and a named contact person.
- Agreement from the parent or carer for sharing information between agencies.
- Any relevant risk assessment information for the teaching venue (home or other).
