Transition Guide: Preparing for Adulthood for parents and carers

This guide sets out how services in Rotherham can work together to support young people with special educational needs and disabilities to have a successful transition from childhood to adulthood.

Growing older and into adulthood is a time of significant change for young people and their families, especially when they have additional needs and disabilities.

This can be a particularly anxious time, which needs more care, planning and preparation than for other young people of similar age. There are many decisions and choices for young people and their families and carers to make about the future related to daily life.

This can be complicated, as support received in children’s services may change when young people reach 18. Whatever your young person's special educational need and/ or disability is, there is a life for them that is full of meaning and opportunity.
