Supporting information about your health condition or disability
As a local authority, we are responsible for making sure that Blue Badges are only issued to people who meet the scheme criteria.
For your application to be successful, we need to be satisfied that awarding you with a Blue badge will enable you to undertake a journey that you would not have otherwise taken.
We also need to be sure that the information you provides evidence that you have a substantial disability that is expected to last for the three-year badge issue period, and one which causes you to:
- Be unable to walk; or
- Experience very considerable difficulty whilst walking, which may include very considerable psychological distress, and / or;
- Be at risk of serious harm when walking; or pose, when walking, a risk of serious harm to any other person
This means that we might need you to obtain additional supporting evidence to help us make a decision about whether or not you are eligible for a Blue Badge.
What will you need to provide?
We will let you know by email if we need you to provide additional supporting information.
If this is the case, we will need you to ask the professional involved in your diagnosis, care or ongoing treatment, to complete a questionnaire.
We will not be able to accept a questionnaire that has been completed by your GP.
What is meant by ‘professional’?
A professional is a person who is involved in the on-going treatment and support of your disability. It should be someone who is familiar with your experience of the condition and the way this affects your day to day life.
Typical examples include:
- Clinical Psychologist
- Educational Psychologist
- Occupational Therapist
What information does the professional need to provide?
If we need you to obtain additional supporting information from a professional, we will provide you with further details when we contact you by email.
We will provide you with a link to an online questionnaire which you can share with your professional. This is the easiest way for them to complete the information. Once they have answered all the questions they can submit the form which is then securely delivered back to us.
Some professionals may prefer to complete a hand written questionnaire. The email we send you will also include a copy of the questionnaire that can be printed off and given to the professional to complete.
What type of questions will you ask?
We will ask the professional to confirm some basic details, such as their name, address, and professional registration number.
We will also ask them to tell us about:
- The disability/disabilities you have been diagnosed with, how substantial this is, and how this will affect you over the next three years.
- Their role in diagnosing your disability/condition.
- Any examples they are aware of where your disability/disabilities have led you to:
- experience very considerable difficulty whilst walking between a vehicle and your destination, or
- be at risk of serious harm, or
- pose a risk of serious harm to another person
- Any coping strategies you have in place – you might for example have support from others, or might have put other things in place to help you better manage your symptoms or behaviours.
