Young person about to turn 21, who is in education or training
The Pathway Plan for a young person who is approaching the age of 21, and who is on a course of education or training that will continue beyond the age of 21, needs to set out the continued support that the young person will receive until their course ends.
If, due to the duration of the course, a further Pathway Plan must be completed then a 21+ Pathway Plan can be used.
The Personal Advisor will continue to review the Pathway Plan until the agreed course of education or training has been completed. Qualifications and future plans will be recorded and the agreed support will end.
Support will end prior to the completion of the programme if it becomes apparent you are pursuing the agreed programme or your circumstances have changed and it is no longer appropriate to continue.
The Personal Adviser will:

inform each Former Relevant Young Person of their entitlement to this support and make a reference to it in the last Pathway Plan before reaching the age of 21

issue a letter at the end of service to each Former Relevant Child detailing this entitlement and the procedure for contacting the local authority should they want to seek assistance in the future
