What the law says we have to do
After 18

Your Social Worker

We are the people who will make decisions about what happens when you’re under 18.

We will:

Make sure the people who are caring for you are helping you learn the skills you will need as an adult.
Work with you to make sure there is a idea as to what your future will hold and make sure the plans are in place to make this happen
Complete the first Needs Assessment and regularly review your Pathway Plan with you until you are 18
Come to see you regularly to make sure you are ok.
Make sure you have all the necessary ID in place that will enable you to access money and a home.

A Needs Assessment is looking at what is happening for you, how you feel, what your hopes are for the future and how we can help you get ready for the future.
