We have worked closely with FS1 Leads and Primary Schools to ascertain what information is most important to support children with transitions into school and have developed a new transition document called ‘My Transition Passport’.
We have condensed the information that is normally produced from Early Years providers through end of term reports into a user friendly and time efficient format. The new document shares information about the children, advising whether they are On Track or Not OnTrack for the 7 areas of learning, along with information about their family, any funding/grants they receive, and services involved with the child and their family.
We have also updated the Cohort Transition document that Early Years providers can use to share information about groups of children attending the same setting.
