3.1a  Those services involved in transition review current transition pathways to see how they meet the needs of autistic young people.   31st October 2021
3.1b.  To develop a universal transition planning passport for all young people 14 -25 31st October 2021
3.2 Ensure the effective coordination and delivery of programmes providing targeted support to autistic people.  This includes initiatives such as Pathways, Work and Health, Building Better Opportunities and Jobcentre Plus work coaches 30th April 2021
3.3 Ensure that advanced planning and coordination of care occurs for autistic young people moving into adult social care.  30th April 2021
3.3a. A clear pathway is developed and is advertised through Rotherham’s Local Offer: 30th April 2021
3.4: Link information and advice to ensure consistency and quality of information through local offer  31st July 2021
3.4a: Raising awareness of what services are available and how to access them 31st July 2021
3.4b: Signpost parents and carers to available sources of support and advice  31st July 2021
3.4c: Local Offer webpage is reviewed and is designed to be as accessible as possible and easier to navigate.    31st July 2021
