Please use this form to request an application form to register for a postal or proxy vote.

We must receive completed Postal vote application forms no later than 5pm, eleven working days before an election.

We must receive completed Proxy vote application forms no later than 5pm, six working days before an election.

BEFORE YOU APPLY

You must be registered to vote before you can be granted a postal or proxy vote.  If you are not registered to vote, you can register online at www.gov.uk/registertovote
