Please use this form to apply for a careworker discount. 

Before starting this form

You will need to:

• Upload proof of qualifying benefit or consent form
• Upload a copy of your payslip
• Provide details of the person receiving care 

If you are unable to supply proof of the qualifying benefit, please download the consent form. This should be completed by the person for whom care is being provided which will enable us to request the information direct from the issuing authority.
