Schools and academies must complete this form for any pupil who falls under the categories:
- Attend school part-time (less than 25 hours a week)
- Receive alternative provision outside the main school site (including online learning)
When to use this form
- Use this form for pupils who receive less than 25 hours a week of educational provision
- Use this form for pupils who receive alternative provision away from the main school site, even if their total educational hours are 25 or more per week
- If a pupil receives both part-time education and alternative provision: only complete the alternative provision return if their entire education package is part-time
