Complete this form to apply to join the Children's Disability Register in Rotherham. A parent or carer can complete the form for any child under the age of 16 and a young person over the age of 16 can complete this independently.
To complete the form information about the child or young person's details and needs will need to be provided.
Please note that the Council will need to contact a young person when they reach the age of 16, as at this point they will need to provide consent for the Council to retain their information on the register. The Council will aim to do this within 6 months of a young person turning 16. If the Council does not receive consent the young person’s entry will be removed from the register by their 17th birthday. Contact will also be made with the young person’s parent or carer to determine their capacity to provide consent.

It will be held and accessible by the Council and information will be shared with the Council’s partner, Rotherham Parent & Carer Forum; both parties will jointly administer the register. Any information provided will not be shared further. Should the Council or Rotherham Parent Carer Forum need to discuss your submission they will contact you via the details you provide.
