Apply to be a chaperone

Please use this form to apply to become a chaperone. You must be aged over 18 to apply.

Documents you will be required to provide

Please ensure you have these to hand before you begin this form.

  • You will be asked to upload a photograph of yourself showing your head and shoulders 
  • If you hold a valid UK passport you will also be required to upload a copy
  • Where applicable to your application you will be asked to a copy of your driving licence and vehicle insurance certificate  
  • If you are a professional chaperone and you have a valid subscription to the DBS update service we will require you to upload a copy of your DBS certificate

You will also need to attend an appointment to provide original evidence of your identity.  

Disclosure & Barring Service (DBS) Check

Following verification of your identity you will be required to complete an online Disclosure & Barring Service (DBS) check.  Details of how to complete this will be provided once your identity has been verified. 

All Chaperones must by law have a Disclosure & Barring Service (DBS) check before they are allowed to supervise other people’s children. Where an applicant is (or wishes to be) a Professional Chaperone a cost will be incurred for the DBS check of £56 unless the applicant has a valid subscription to the DBS update service. Where an applicant is a Voluntary Chaperone (unpaid) there is no charge.

It may take up to 12 weeks for your chaperone licence to be issued.
