Please use this form to apply for the child to be assessed for specialist home to school travel assistance.
The child may qualify for additional support travelling to and from school/unit if they fulfil all of the following criteria:
- Are aged up to 16
- Live in the Rotherham area
- Have an Education, Health & Care Plan and have physical or medical needs/disabilities which prevent them from travelling independently on public transport.
Where relevant you will be asked to upload a copy of the child’s medical report as evidence of any medical or vulnerability conditions. You will also need to provide your child’s height (cm) and weight (kg). Please ensure you have all this information before starting this form.
