Autism Information and Advice Referral

Child/Young person must be registered with a Rotherham GP Practice to be eligible for referral to this team.

This referral form should only be used by RDASH CAMHS or Child Development Centre practitioners, who are referring parents/carers of a newly diagnosed child/young person who:

• Has been given an Autism Spectrum Disorder diagnosis and
• is registered with a Rotherham GP Practice.
