Hospital or institution discharge
If you are staying in hospital or an institution, and you think you could become homeless when you are released, it is important that you speak to the hospital discharge team.
It is important that you contact us as well as soon as you know that you could become homeless. We can work with you, medical staff, hospital discharge team and other services that you work with, to ensure that you can move into accommodation that is suitable for your needs.
If you are going to be homeless upon release, then the hospital or institution have a duty to refer you to a local authority of your choice so you can receive assistance.
