Purpose of this form

This form is intended for those wishing to apply for rehousing based on a medical need.

Before you apply

Before you look for a council home you must first register your details with Keychoices.

Once you have registered you will receive a Unique reference number. This will allow you to log in to the housing online service to complete a Housing Application Registration Form.

Information you will need to apply

In order to apply for medical rehousing, you will need:

  • Your Housing Application reference number from Key Choices
  • Your personal details (including your national insurance number)
  • Information about your medication and medical conditions (including evidence, where possible)
  • Your current residence (including type and any adaptations)
  • The details of anyone to be rehoused with you
  • The address of anyone that provides help or support to you
  • Information of any armed forces service
  • Details of current employment
  • Details of any benefits received
  • Information about anyone that provides you with help and support that has a power of attorney

Please make sure you have all of this information to hand before you begin the form. For security reasons, each form page times out after 10 minutes.
