What is Enablement?
The Enablement Service can help you to regain living skills, confidence, and mobility. Some people work with the Enablement Service when they have been in hospital. Others need support to help them to continue living at home.
The Enablement Service can:
- Provide guidance about daily activities, such as personal care and dressing, and preparing your own meals.
- Arrange for assistive living technology, such as alarms that you wear around your neck or wrist to alert someone if you fall.
- Arrange for equipment like perching stools that will make activities easier and safer.
Who is Enablement for?
Enablement can help people who need support with daily activities because, for different reasons, they are finding it more difficult to do them.
The service is for anyone over the age of 18, who meets the eligibility criteria and doesn’t have formal care and support from Rotherham Council’s Adult Social Care, and who would benefit from a period of short-term reablement to maximise their independence.
For some people Enablement may not be the right service, for example, those already accessing formal care and support from a long-term provider or people who have serious illnesses requiring specialist care and support. If you or a family member is in this situation, we will always ensure that you are connected to the right team as quickly as possible.
What does Enablement involve?
If Enablement is identified as the next step for you, then an Enablement Assessment and Plan will be completed with you, highlighting the areas of your life you need support with and want to focus on to achieve your personal outcomes and goals.
Enablement will be provided for up to six weeks. Your progress will be reviewed after each support session and the type of support may change as you gain in confidence and progress towards being able to complete a task independently.
Once you have reached your maximum independence a further discussion will be arranged with you and your family about the next steps and a provisional end date agreed.
Some people may only need support for a few days while others may need the full six weeks of support.
Some people may still require a full assessment to see if they are eligible for ongoing care and support. The assessment team linked to the Enablement Service will arrange to complete an assessment with you.
Will I have to pay for Enablement?
This service is free for up to six weeks. The length of time you receive enablement may change, such as if we assess that your independence has been reached, or that you need ongoing care and support. If this is the case, you will need a financial assessment and may have to contribute to the cost of your future services.
