Going into hospital
If you go into hospital, you will need to tell anyone who provides support for you. They can pause the services until you return home.
Returning home from hospital
You may need some short-term support at home once you leave hospital. A member of the Integrated Discharge Team will contact you and your family to talk about this. This support might come from the Enablement Service.
We can complete a social care assessment to see if you are eligible for care and support from Adult Social Care.
If you already receive care, you will need to tell your care provider when you are going to return home. They can arrange for the services to start again.
Some people are unable to return home and need a short period of recovery. A small number of care homes can provide this.
