Apply for specialist home to school travel assistance
The child/young person may qualify for additional support travelling to and from school if they fulfil all of the following criteria:-
- Are aged up to 16
- Live in the Rotherham area.
- Have an Education, Health & Care Plan and have physical or medical needs or disabilities which prevent them from travelling independently on public transport.
Where relevant you will be asked to upload a copy of the child/young person’s medical report as evidence of any medical or vulnerability conditions. You will also need to provide the child/young person’s height (cm) and weight (kg). Please ensure you have all this information before starting the form.
